In order to confirm the identity of the person requesting disclosure, etc., we will conduct a verification call over the phone. However, if verification cannot be conducted over the phone, you may be asked to present a copy of your driver's license, resident card, health insurance card, etc.

4.Request for disclosure, etc. by a representative

If you wish to delegate the request for disclosure, etc. to an agent, please enclose the following documents in addition to the request form for disclosure, etc. of personal information.

  2. Power of attorney (The person in question must affix a seal to the power of attorney and attach the seal registration certificate for that seal. If the agent is a legal representative such as a parent, they may submit a certified copy or extract of their family register or resident registration card that shows their relationship to the person in question instead of a power of attorney.)

If you request disclosure of your personal information and notification of the purpose of use, a fee of 500 yen (excluding tax) will be charged for each request.
To pay the fee, please transfer 500 yen (excluding tax) by bank transfer.
*When verifying your identity, we will provide you with the bank account information for the transfer.

Please note that if the fee is insufficient or if we are unable to confirm your payment, we will be unable to disclose or notify you of the purpose of use.
